Part of my somatic sex education training focused on this, and I was amazed at how much I learned. To make a very brief summary, scar tissue grows after surgeries, wounds, and sometimes even inflammation and trauma. It is a connective tissue that develops with different fibers than the body creates without scars. It can cause adhesions, meaning multiple layers of tissue stick together instead of sliding past each other (for example, after a C-section, the uterine muscle, fascia, and abdominal muscles stick together), and it can also cause changes in sensation, such as numbness and pain. If you’d like to learn more about scar tissue, I talk more about it in my recorded class, La Sagrada Pelvis. Personally, I was completely blown away by the knowledge that many people with vulvas experience changes in their sensations due to pelvic or genital scarring after vaginal births, cesarean sections, or surgeries, and that no one addresses these changes by focusing on pleasure.

Western medicine offers few solutions for scar tissue, but it can be treated very well with manual therapy. In Montreal, I was able to meet with two colleagues from somatic sex education and a massage therapist trained in scar work to discuss our scar-touching techniques. I continue to learn with every scar I touch, and I’m passionate about continuing to do so.

What kind of scars have I supported?

Top surgery scars

Genital scars

Gender-affirming vaginoplasty

Pelvic scars from c-sections, hysterectomies, ectopic pregnancy surgeries

Mastectomy and implants scars due to breast cancer

In addition to the physical aspect, manual scar therapy can generate many emotions and memories, which I think is important to explore in a safe environment to integrate these experiences. Even for scars from gender-affirming surgeries, it’s very common to experience conflicting emotions, such as euphoria and grief, comfort and disconnection at the same time. It also holds a lot of tension, and physiologically, many emotions can be somatized as pelvic tension. I believe we lack education and care for our pelvis!

It’s very common to experience pelvic pain, which can limit our experience of sexual pleasure and well-being. This pain can have physiological causes, such as scar tissue after childbirth, abortion, sexual abuse, or infections; structural causes such as posture, weakened or tense pelvic muscles, and scar tissue adhesions in the abdomen; or emotional/psychosomatic causes such as shame, social taboos related to sexuality (sometimes unconscious), grief, etc.

The pelvic work I offer always begins without touch, sharing exercises, anatomical explanations, and body awareness.
